Caring for our people

Ambition: Caring for those who need it and supporting everyone to live healthy and independent lives.

Objective 3. Help residents live long and healthy lives and provide support to those who need help.

• Health improvement – Prevent future illness and reduce the need for greater care by supporting residents to live healthier lives, remain active and independent.
• Support those who need help - Practical measures that make a real difference to people’s lives.
• Equitable access to services and outcomes - Reflect the different characteristics of our communities, including rural communities, when we assess need and commission services.

Objective 4. Ensure effective, high-quality, sustainable care and support services that enable vulnerable adults to live independent lives in a place they call home.

• Living independently to prevent the need for greater care - Enable older and other vulnerable adults to live independently through the availability of a range of housing to meet their needs and the right care and support services.
• Care Villages and accommodation with care – Develop models such as Care Villages and extra care housing where they meet local need, promote independence and help people remain in their communities for longer; improving wellbeing and reducing demand for higher-level care.
• Sustainable care market and value for money - Ensure quality providers can deliver excellent services. Effective health & care services that justify every pound spent and focus on targeted support, de-escalation and provision of in-county care wherever possible.
